Program Spotlight . . .

Permanent Supportive Housing for the Chronically Homeless

PROGRAM:  The Permanent Supportive Housing program provides housing for adults who have struggled in the past with maintaining housing and are in need of a supportive living arrangement.   The program is designed to keep individuals in housing while working on their life-recovery goals.

TARGET POPULATION:  (12) Adults who meet the HUD definition of chronically homeless: homeless for one-year continuously or have had four episodes of homelessness in the past three years, not have custody of dependent children, and have a disability.  Disabilities include mental, physical, developmental, and addictions. 

LENGTH OF STAY:  The length of stay is unlimited and restricted only by a participant’s compliance with program guidelines.

CASE MANAGEMENT SERVICES INCLUDE:  The guidance and support of a case manager, accountability, community networking, life-skills development, career guidance, employment services, substance abuse counseling, medical and mental health referrals, transportation, transitional housing, permanent housing assistance, and linkages to government and community agencies.  Personal growth is encouraged and supported in all areas of life including: physical, mental, emotional, financial, educational, social and spiritual.

STAFF: One case manager is assigned to the Permanent Supportive Housing program participants. She maintains regular contact with participants, monitors their progress, adjusts their Life Recovery Plans as needed, and coordinates the community support services needed to achieve their goals. A registered nurse, certified drug and alcohol counselor, mental health worker and Global Career Development facilitator on staff at Carpenter’s Place are also available to assist program participants.
