COLLABORATION:
The majority of our Guests have very severe and urgent needs in almost every area of life.
The needs are far too diverse and require far too many specialists for us to be effective
with anything other than that of a concurrent wrap-around services approach. Therefore,
the CP Model aggressively seeks to develop service collaborations and alliances, both in
and out of the local community, which will allow the core issues in all 14 areas of an
individual's life to be addressed at the same time.
We have invested much time and energy in developing and utilizing any and all of the
available resources in Rockford that may facilitate our mission to our Guests. We
systematically visit the sites of collaborative agencies and invite them to visit with us
in an effort to develop solid working relationships. There are over 400 agencies and
organizations in our resource database. We have utilized resources in at least eight
different states.
NON-DUPLICATION OF SERVICES:
CP also strives to avoid duplication of services already available in the community. If a
required service is not currently available, or is not available in sufficient capacity to
meet the need, CP will work in a collaborative manner to fill that need. CP has a track
record of forging and participating in community collaborations to address gaps in
services for the homeless and to increase the efficiency of service delivery in this
community.
Our Homeless Mental Health Access Project (HMHAP) is the perfect
example of how several service providers came together to address a
gap in our community. The HMHAP project is a collaborative between
CP, Janet Wattles Mental Health Center , Red Cross Shelter, and
Crusader Clinic. This program expands access to mental health
services and medication for homeless individuals who otherwise would
not have been eligible for such services. Because of this
collaborative, the HMHAP Case Manager can ‘open doors' on behalf of
the homeless at all four agencies. The client benefits from the
cooperation and each agency is able to claim a portion of the
success of the program.
